AM shifts: 5 days a week, 8 hours per shift (40 hours/week). Monday to Friday.

Job Description

We are seeking a detail-oriented and compassionate Licensed Vocational Nurse (LVN) to provide high-quality home health care. The successful candidate will cover a designated area that includes Atlanta, Midtown, and Buckhead, as well as potentially assisting in neighboring territories within a 60-mile radius. Responsibilities include routine visits and maintaining a high standard of patient care.

Responsibilities

• Conduct routine visits to patients in their homes.
• Provide nursing care and support according to established protocols.
• Maintain accurate patient records and documentation.
• Coordinate with healthcare professionals and assist in the development of patient care plans.

Required Experience / Certifications / Licensure

• Current LVN license.
• Recent home health experience is required.
• Experience with HomeCare HomeBase (HCHB) is preferred; candidates without this experience may be considered at a reduced rate for training.
• Ability to achieve 32 points per week through routine visits.
